An Invitation to Professionals

Providers and patients work best together when the patient has available and shares accurate details of her or his health history and is able to document that she or he has adhered to the health goals and instructions that have been mutually decided upon. Providers often have the use of electronic health records (EHRs) to assist with tracking examination, laboratory and imaging-generated data, but there has been little to assist patients and their families in tracking and sharing health-related information, adherence to medication and life-style regimens, and to visually depict the effect of these interventions on health goals. Such tracking becomes even more important in patients with multiple, often chronic health problems, requiring close monitoring of multiple medications and instructions from multiple providers.

The SmartPHR® Health Aware mobile app is designed to maximize patients’ ability to record health goals mutually decided upon with their providers and to follow these along with the impact of new or changed medications and life-style changes with clear graphics, emphasizing trends over time. These goal graphics are easily shared with providers, when reviewing the impact of prior instructions, considering the need for changes in dose or type of medication, and a new course of action or action plan.

The app also keeps a record of patients' basic health information, such as current and past illnesses and health problems, medications and dosages, other interventions, allergies and drug sensitivities, emergency contact information, as well as provider and insurance contact information. In addition, the app has a function that serves as a daily reminder to follow and document specific instructions.
