Care for Chronic Conditions

Security. The Smart PHR™ System is a consumer-controlled, interoperable personal health record system that employs the security practices characteristic of online banking systems including data encryption in storage and transmission and daily penetration testing and vulnerability repairs.

Privacy. The Smart PHR™ System gives consumers, and their designated account administrators, unique tools for granting, managing, and auditing user access privileges for family caregivers, health and human service providers, and other trusted individuals. After account administrators (consumers or delegates) log into Smart PHR client accounts, they follow this path: Dashboard>Vital Information>Account Administration>Manage Privileges. For more information about the obligatory Terms & Conditions of Smart PHR System use, click here.

Health Insurance Portability and Accountability Act (HIPAA). HIPAA’s Security and Privacy Rules regulate the electronic exchange of information between covered entities including healthcare providers, health insurance payers, pharmacies, and labs via information systems such as doctors’ electronic health record systems (EHRs). HIPAA does not cover data exchange by consumers via personal health records (PHRs). Nevertheless, we voluntarily operate the Smart PHR™ System in compliance with HIPAA Security and Privacy Rules.

Consumer-first mission. Unlike other PHR vendors, we do not reuse any consumer data to market third-party products; mine or resell consumer data to third parties; access consumer accounts without explicit authenticated requests from consumers; or sell ad space on our website. Our mission is to do well by doing good for consumers like us.

Health Information Technology Standards (HITSP). Since the establishment of HITSP, Prosocial’s management has been actively involved in this nationwide effort to set standards for secure, private, and confidential information exchange among PHR and EHR systems. We are committed to continuously upgrading The Smart PHR™ System so that it leads the way in standards adherence and in utility for consumers, family caregivers, and clinicians.

Providers’ Consent to Share Information & HIPAA Privacy Practices Documents. Consumers or their designees may sign, scan, and upload providers’ own consent and HIPAA privacy forms. To view these documents, log in to your Smart PHR client account and follow this path: Dashboard>Vital Information>Consent to Share Information.
