A Smart PHR account creates a comprehensive lifelong record of a consumer’s vital information and documents. A Smart PHR aggregates information scattered in doctors’ paper and electronic files with information entered online by the consumer and his or her authorized family caregivers and clinicians.
A Smart PHR account is accessible 24x7 to all consumer-authorized users from any web-connected device including cell phones without software downloads, upgrades, or maintenance. Users engage in cloud computing to access the Smart PHR Platform on our secure servers.
A Smart PHR account is not tethered to a particular organization’s or provider’s electronic health record system (EHR).
The Smart PHR Platform is capable of exchanging information with any EHR system that is compliant with HITSP interoperability specifications (link to ansi.org/standards_activities/standards_boards_panels/hisb/hitsp.aspx )and IHE technical frameworks (link to ihe.net/Technical_Framework/). Consumers may employ their Smart PHR accounts to authorize automated exchange of information with interoperable PHR and EHR systems.
A Smart PHR account has all the capabilities of a patient’s record in a provider’s EHR system. In an EHR system, the provider’s appointed system administrator manages user access privileges for the system as a whole. With Smart PHRs, consumers (or their designated account administrators) control role-, relationship-, and content-based access privileges. Designated account administrators may be family caregivers or healthcare provider representatives such as patient navigators.
Care coordination, disease management, and decision support capabilities are built in to Smart PHR Life Agent™ Care Plans. These tools are available to consumers with chronic conditions, their family caregivers, and their EHR-equipped and EHR-deprived health and human service providers.
